Knee osteoarthritis has become one of the leading causes of disability in older adults. Besides knee replacement, there are no curative treatments for knee arthritis, so persistent pain is commonly treated with opioids, acetaminophen, and nonsteroidal anti-inflammatory drugs (NSAIDs).5
Researchers in a 2019 study of adults aged 65-75 with knee osteoarthritis sought to test the efficacy of a dietary intervention as an alternative form of pain management because of the many unpleasant side effects of drugs.
Participants were asked to follow either a low carbohydrate diet, a low-fat diet, or continue to eat as usual over 12 weeks. Functional pain, self-reported pain, quality of life, and depression were assessed every three weeks. Serum from before and after the diet intervention was analyzed for oxidative stress.
The results showed the low carbohydrate diet reduced pain intensity and unpleasantness in some functional pain tasks, as well as self-reported pain compared with the low-fat diet and the control group of non-dieters. The low carb diet also significantly reduced oxidative stress and the adipokine leptin compared with the other groups. Reduction in oxidative stress was related to reduced functional pain.
Through their evidence, the authors concluded that their low carbohydrate diet intervention could provide relief from pain and be an opioid alternative. The study was published in the Journal Pain Medicine, March 13, 2019.5

A 2017 study that incorporated the best diet shake for twice-daily (breakfast and lunch) meal replacements was effective in achieving clinically meaningful weight and fat loss while not losing lean body mass. The study was the longest (six months) and largest of its kind. It discovered that a combination of 40 minutes twice weekly of resistance/aerobic exercise in combination with a specific structured meal replacement program was more effective than exercise alone at producing positive effects on body composition (reduction of fat mass, maintenance of lean body mass).
Subjects of programs without nutritional support and exercise typically lose 3.5 pounds of lean body mass during similar study periods. However, participants in this study gained 3.7 pounds of lean body mass and experienced significant improvements in blood pressure and hemoglobin-A1c during the study period. This preferred weight loss manifests in lost inches more significantly than lost pounds.
The study was published in the Journal of Exercise Physiology in February 2017. Protein powders are often contaminated with heavy metals and could pose a health risk, according to investigative tests performed in 2018 and 2010.7
Consumer Reports analyzed the levels of contamination in the top-selling protein powders and found 100% contained detectable levels of heavy metals such as arsenic, cadmium, mercury, lead, and bisphenol-A (BPA), according to the report, Arsenic, Lead Found in Popular Protein Supplements by Jesse Hirsch on March 12, 2018.8
Virtually all the 134 products tested from 52 brands contained at least one heavy metal and 55 percent tested positive for BPA. These substances have been linked to cancer, brain damage, and reproductive issues. The products were screened for over 130 toxins and were tested and reviewed by a third-party analytical chemistry laboratory.
“These toxins accumulate in your body and can stay there for years,” says Tunde Akinleye, a test program leader in Consumer Reports Food Safety Division. “Frequent consumption of foods that contain them can have adverse health effects over the long run,” she said in the article.8
This is not the first research that has shown high contaminant levels in such products. A 2010 Consumer Reports study detected arsenic, cadmium, lead and/or mercury in samples of all the 15 powders tested.7
A lack of consistent, organized raw material testing by U.S. manufacturers, weak industry regulations, and cheap foreign ingredients have all contributed to these findings.
Here are some tips when trying to find the best diet shake. Look for a protein that is in powder form (not ready-to-drink), tested for purity and quality, with no artificial ingredients, additives or other surprises says Steve Hertzler, Ph.D., Abbott Nutrition senior research scientist and dietitian in a Livestrong article.9
How can consumers know it’s tested for purity and quality? Look for a product that is checked for banned substances and has label claims verified via a National Science Foundation (NSF) or U.S. Pharmacopeia (USP) type certification, or other label claims specifically related to purity.
